Background: Cardiopulmonary bypass in children results in considerable water retention, especially in neonates and small infants. Dilution of plasma proteins increases water loss into the extravascular compartments. Excessive total body water may prolong ventilatory support and may contribute to a prolongation of intensive care convalescence. Unlabelled: In a 2 year period seven patients who presented with stridor, without respiratory compromise, and three patients without obstructive symptoms were prospectively selected, and underwent MRI. In eight patients with a vascular ring and a pulmonary sling, MRI delineated the vascular abnormality and normal great vessels were found in two patients. A 42-year-old man underwent assumed total surgical repair of an acyanotic tetralogy of Fallot: a perimembranous ventricular septal defect (VSD) was closed with a dacron patch and myectomy of the infundibulum of the right ventricular outflow tract was performed. Reexamination eight years later revealed a large recurrent VSD and by surprise a narrow supravalvular ridge above rudimentary pulmonary cusps in the pulmonary trunk, leading to a pressure drop of 70 mmHg across the supravalvular stenosis.... Tissue degeneration reduces the durability of cryopreserved homografts. Earlier studies indicated that the presence of fibroblasts in homograft leaflets may contribute to increased valve longevity. A 5-month-old female infant with Down syndrome underwent surgical repair of a complete atrioventricular septal defect and a patent ductus arteriosus. The postoperative course was complicated by life-threatening pulmonary hypertensive crises which did not respond to conventional vasodilating therapy. From 1958 to 1992, 105 patients were operated for discrete (or membranous) subaortic stenosis (DSAS). Our surgical techniques changed during this period. Before 1980 DSAS was removed by a sharp excision leaving the part on the mitral valve untouched (n = 58). After 1980 blunt dissection was used to enucleate DSAS completely (n = 47). Clinical use of the stentless bioprosthesis has not yet been accepted world-wide. Experimental studies are scarce. In a study in growing pigs, 23 aortic valve replacements were performed (7 stented bioprostheses (STB), 10 stentless bioprostheses (SLB) and 6 cryopreserved homografts (CAH)). This report describes the experience of the Leiden University Hospital with the arterial switch operation for transposition of the great arteries, with and without ventricular septal defect, and for the Taussig-Bing-anomaly.
